Name and address of drivers
Driver license numbers and state of issue
Vehicle license plate numbers and state of issue
Make of vehicles
Model and year of vehicles
Damage to each vehicle
Insurance company names and policy numbers
Names and addresses of passengers
Witnesses names and contact information,
if present
Precise location of the crash: street, city, state
Weather and road conditions
Police report: officer name/badge,
report number
Any other actions required by your state
CRASH REPORTING
In case a crash occurs:

STOP.
X
If you can, move your vehicle off the road
so that you do not block traffic.

Do not move an injured person unless necessary.
X
Call 9-1-1, if needed.
X
Get the names and addresses of all people involved
in the crash and any witnesses.
X
Take pictures of registrations, vehicles,
street name, weather/road conditions,
licenses, house numbers/location, etc.
